Participation Release I wish to participate as a volunteer for Vacation Bible School at Gibbsville Church or for my child(ren) to participate as a volunteer. I am aware of any special dangers and risks inherent in participation including physical injury, or other consequences arising or resulting from volunteering during Vacation Bible School. I understand that all reasonable safety precautions will be taken by the leaders and other volunteers. I hereby release Gibbsville Church, its staff, volunteers, and leaders from liability in the event of an accident or injury. In the event of an emergency, I authorize the church and its representatives to obtain medical treatment for my child(ren), including emergency medical or surgical care, if I cannot be reached. I understand that I am responsible for any medical expenses incurred. I certify that the information provided on this form is accurate and complete.

Photo Release I give permission for myself or child, serving as a volunteer, to be photographed and/or videotaped during VBS at Gibbsville Church. I understand that these images may be used in church slideshow presentations, the church website, social media, or printed publications for promotional purposes but no individual or child will be identified by name.
